Track wellness visits and outreach
Draft — 2026-09-26
Drafted from the screen's code on main (kareIntelligence 7455702). Not yet reviewed against a live session. Roles listed are the defaults the app's role matrix (@collabkare/ability 0.12.0) gives this screen's route; your organization's role overrides can change them.
Quality & Risk → Wellness Visits tracks Medicare wellness visit (MWV) compliance over the attributed panel on a 365-day cycle, and finds who to contact first.
Tabs
- Current Opportunities — eligible members, compliance (members with an MWV in the trailing 365 days), the due backlog, members with no prior MWV, MWV-coded spend, the panel by due bucket (overdue, due now 0–30 days, upcoming 31–90 days, current), and a Clinical measures (CR lane) strip.
- Provider Performance — MWV compliance by PCP against the filtered panel's blended compliance, and the share of each panel's last MWVs delivered by its own PCP.
- Trending — visits by month year over year, with rolling compliance.
- Patient Roster — the actionable list, most-overdue first. Filter by bucket.
- Patient Level — bucket statistics for the population in scope.
Acting on the roster
- Select members on Patient Roster.
- Choose Add n to outreach worklist to create work items in kareManager's Wellness Visit Outreach.
- Or, where outreach drafting is switched on, choose Draft outreach on one member to create a draft for review in Outreach drafts — nothing is sent.
Things to know
- The compliance line on Provider Performance is an internal benchmark, not a HEDIS or national one.
- If the worklist service is unavailable, intake is disabled and nothing is created — a write never silently succeeds.
- Providers appear as PCP IDs.
