KareManager overview

KareManager is CollabKare's care management workspace for population health and care coordination teams. It is where you find your members, review their full record, and document care management work — care plans, assessments, transitions of care, medication reviews, and utilization reviews.

Who uses KareManager

Your access is based on the role your administrator assigned to you:

  • Nurse Care Manager — care plans, SDOH assessments, transitions of care, disease program enrollment, care gaps
  • Social Worker / Community Health Worker / Service Coordinator — SDOH assessments, care plans, community referrals
  • Care Coordinator / Care Navigator — care plans, referrals, transitions of care
  • Behavioral Health Clinician — crisis plans, care plans, assessments
  • Pharmacist / MTM Specialist — medication reviews (MTM), drug utilization reviews
  • Utilization Management Nurse — utilization case reviews, readmission review, ER diversion, post-acute care
  • Clinical Document Specialist — documentation review
  • Claim Specialist — claims and financials
  • Pop Health Manager — population dashboards, outreach campaigns, surveys, reports, oversight of most areas

When you sign in you pick your role (and your workspace, if you belong to more than one). The left menu and the actions you see are tailored to that role.

  • Members — Member Search, Disease Programs, Care Gap Worklist, SDOH Assessment Queue, Care Plans, Transitions of Care, Remote Patient Monitoring
  • Quality & Outreach — Outreach Campaigns, Member Outreach, Surveys, Member Education Library
  • Utilization Management — Case Reviews, Readmission Review, ER Diversion, Post-Acute Care
  • Pharmacy — Medication Review (MTM), Drug Utilization Reviews
  • Care Coordination — Referral Tracker, Care Navigation Queue
  • Community Supports — Service Referrals & Tracking
  • Workflows & Admin — Policies, Attribution, Audit Logs, SMART App Gallery, Help & Support

Most day-to-day work starts from Members → Member Search: open a member's workspace, then use the tabs and Role Actions to document your work.
