Write an MTM consultation note

Document a medication therapy management (MTM) consult — a comprehensive or targeted medication review — and send your recommendations to the prescriber.

Before you start

  • You need the Pharmacist / MTM Specialist role.
  • In the left menu, go to Pharmacy → Medication Review (MTM), or start from a member's workspace via Role Actions → MTM Consultation Note.
  • Review the member's Medications tab first so your findings reflect the current med list.

Steps

  1. On the Medication Review (MTM) page, click New MTM Review.
  2. Find and select the member in the search that opens. The MTM Consultation Note form opens.
  3. Choose the Review type:
    • CMR — Comprehensive Medication Review
    • TMR — Targeted Medication Review
  4. Optionally enter an Adherence score (for example, "PDC 78%").
  5. In Findings, document drug-related problems, gaps, interactions, dosing concerns, and formulary alternatives.
  6. In Recommendations to prescriber, write your specific recommendations (for example, "Switch to lisinopril 20 mg daily; discontinue duplicate ARB").
  7. Click Save MTM Note. You return to the member's Face-Sheet.

When saved, the note is filed on the member's record, and if you entered recommendations, they are sent to the care team as an MTM recommendation message.

Tips / Troubleshooting

  • You must enter either findings or recommendations — the form won't save if both are empty.
  • The saved note appears on the member's Documents tab.
  • For drug utilization review cases assigned to you, use Pharmacy → Drug Utilization Reviews instead — that's a case worklist, not a consult note (see "Work utilization and DUR case reviews").
