PopulationHealth overview

PopulationHealth is the analytics app for teams who look after whole patient populations rather than one patient at a time. It brings quality performance, risk scores, care gaps, cost, pharmacy, and health-equity views into one place so you can spot where your population needs attention.

Who uses it​

Access is role-based. Roles your administrator can assign include Program Administrator, Pop Health, Quality, Cost of Care, Risk Adjustment, Provider Assessment, Pharmacy, Health Equity, and System Support. After you sign in, you pick your practice and role on the Select Practice / Select Your Role screen and press Continue to Dashboard. The left menu then shows only the areas your role can use.

Main areas (left menu)​

  • Home — Dashboard, Today's Outlook (today's schedule and opportunities), and Reports.
  • Quality & Analytics — Quality Measures, Clinical Measures, Care Gaps Analysis, and EHR Quality Metrics.
  • Risk Adjustment — HCC Dashboard and Chronic Disease Map.
  • Utilization — Utilization Dashboard.
  • Disease Registries — Disease Registry (diabetes, kidney disease, hypertension, oncology).
  • Cost of Care — Cost of Care Dashboard and Geographic Analysis.
  • Population Health — HEDIS Engine and Geographic Analysis.
  • Provider Assessment — Provider Assessment Dashboard.
  • Pharmacy — Pharmacy Dashboard.
  • Health Equity — Health Equity Dashboard.
  • Patient Management — Patient 360.
  • Operations — Operational Dashboard, System Monitor, Design Studio, Database, and Settings (admin roles).
  • SMART Apps — App Gallery.

About the data you see​

The app currently ships with showcase data, and an amber Demo data notice appears at the top of pages while that mode is active. All screens work normally — you can browse, filter, sort, and drill down — but the patients, metrics, and measures shown are sample data until your administrator connects your organization's live data.
