Review the HCC Risk Dashboard
The HCC Risk Dashboard ranks your patients by risk score so risk-adjustment and quality teams can find high-risk patients and recapture opportunities quickly.
Before you start
- You need the Risk Adjustment, Quality, or Program Administrator role.
- In the left menu, go to Risk Adjustment → HCC Dashboard.
Steps
- Read the summary cards: Total Patients, Avg HCC Score, High Risk (≥2.0), and Recapture Opportunity (estimated dollar value).
- Narrow the patient list with the Filters panel on the left:
- HCC Score Range — enter Min and Max scores (0 to 4).
- Provider — check one or more providers.
- Insurance — check one or more plans.
- Sort the patient table by clicking a column heading; click again to flip the sort order. Risk tiers are labeled from Very Low up to Critical.
- Adjust how many rows you see with the Per page selector, and page through results with the pagination controls at the bottom.
- To contact patients, tick the checkbox next to each one (or the header checkbox to select the whole page), then press Send Messenger in the blue bar that appears above the table.
Tips / Troubleshooting
- A quick way to build a recapture worklist: set HCC Score Range Min to 2.0, sort by score descending, and work from the top.
- If the table looks unexpectedly short, clear the provider and insurance checkboxes — filters persist while you stay on the page.
- Related view: Quality & Analytics → Quality Measures has an HCC Analytics tab (compliance by measure) and an HCC Gaps tab (patient-level gaps with recapture value); rows on the analytics tab open a measure drill-down.
