  • Risk scores and coding gaps — kareIntelligence Risk Capture shows RAF scores, open HCC coding gaps and the recapture opportunity by PCP.
  • Working recapture items — kareManager's HCC Recapture Queue (under CDI & Risk Adjustment) and the Care Gaps & Outreach worklist hold HCC recapture cases to assign, query and close.
