Use the Disease Registry

PopulationHealth was switched off on 2026-09-25 and is no longer reachable, so its Disease Registry (diabetes, kidney disease, hypertension and oncology tabs) is gone.

Where to look now​

There is no like-for-like condition registry yet. The closest views are in kareIntelligence:

  • Population Insights — risk bands, demographics and captured-HCC prevalence for the attributed population. It says plainly that chronic-condition prevalence beyond the CMS-HCC categories is not in this build.
  • Cohorts — save a filtered population and re-measure it after every pipeline run.
