Use the Disease Registry

The Disease Registry tracks your population by condition — diabetes, chronic kidney disease, hypertension, and oncology — with control charts, trends, and a searchable patient list for each.

Before you start

  • You need one of these roles: Quality, Pop Health, Program Administrator, Risk Adjustment, Provider Assessment, or Health Equity.
  • In the left menu, go to Disease Registries → Disease Registry.

Steps

  1. Pick a condition tab at the top: Diabetes, Chronic Kidney Disease, Hypertension, or Oncology.
  2. Review the charts for that condition:
    • DiabetesHbA1c Control Distribution (Well Controlled through Very Poor) and HbA1c Trend (6 Quarters).
    • Chronic Kidney DiseaseCKD Stage Distribution (Stage 1 through Stage 5) and eGFR Trend (6 Quarters).
    • HypertensionBP Category Distribution (Normal, Elevated, Stage 1, Stage 2, Crisis) and BP Trend (6 Quarters).
    • OncologyCancer Type Distribution and Treatment Outcomes (Complete Remission through Progressive).
  3. In the Patient Registry card, type in Search by patient name... to find a specific patient in that registry, and review their latest values in the table.
  4. On the Diabetes tab, check Screening Alerts for patients who are due or overdue for testing.
  5. Use the Export button at the top right to download the registry view.

Tips / Troubleshooting

  • Switching condition tabs clears your search box — re-enter the patient name after changing tabs.
  • While the amber Demo data notice is showing, the registry patients and lab trends are sample data; the same screens apply once your administrator connects live data.
