Build a care plan
Create a structured care plan with conditions, recommended services, and measurable goals using the six-step Care Plan Builder.
Before you start
- You need a care role that can create care plans (Nurse Care Manager, Social Worker, Behavioral Health Clinician, Care Coordinator, or Care Navigator).
- In the left menu, go to Members → Care Plans. This page lists existing care plans with their status, progress, goals, and interventions.
Steps
- Click New Care Plan at the top right. The Care Plan Builder opens with a step list on the left and a progress bar in the header.
- Step 1 — Patient: search for and select the member the plan is for.
- Step 2 — Conditions: choose the conditions from the member's record that this plan will address.
- Step 3 — Assessment: review the member's recent results and screening findings.
- Step 4 — Services: pick recommended services and programs to include in the plan.
- Step 5 — Goals: set goals and actions for the plan (for example, "Maintain BP below 130/80" with a target date).
- Step 6 — Review: check the full plan summary, then click Publish Care Plan. A Saved confirmation appears in the header.
- The new plan now appears on the Care Plans list, where you can reopen it or track goal completion.
Tips / Troubleshooting
- Use the Next/Back arrows or the step list to move between steps; you can't advance until the current step's required choices are made.
- For a quick plan while you're already in a member's workspace, use the Care Plan Builder entry under Role Actions — a single-page form with a title, dates, and goals, saved with Save Care Plan.
- Click Refresh on the Care Plans list if a just-published plan doesn't appear immediately.
