Enroll a member in a disease program

Enroll a member into a disease management program (for example, Diabetes Management or CHF Management) and automatically create the onboarding outreach task.

Before you start

  • You need a role that can enroll members in programs (Pop Health Manager or Nurse Care Manager).
  • Open the member first: go to Members → Member Search, find the member, and open their workspace.

Steps

  1. In the member's workspace tab bar, find the Role Actions group and click Enroll in Disease Program.
  2. Choose the Program from the list. Available programs include:
    • Diabetes Management, CHF Management, COPD Management, Asthma Management
    • CAD / Cardiac Rehab, CKD Care Pathway
    • Behavioral Health Program, Maternity Program
    • Transitions of Care, Remote Patient Monitoring
  3. Set the Enrollment date (defaults to today).
  4. Optionally enter the Assigned case manager (for example, "Maria Lopez RN").
  5. Add Enrollment notes — reason for enrollment, baseline status, and target outcomes.
  6. Click Enroll Patient. The system creates an active program episode on the member's record and a "Welcome / onboarding call" task for the care team, then returns you to the member's Face-Sheet.

Tips / Troubleshooting

  • The new enrollment appears on the member's Episodes of Care tab, and the onboarding call appears under Tasks & To-Do.
  • If you don't see Enroll in Disease Program under Role Actions, your current role doesn't allow enrollments — switch to the correct role at sign-in or ask your administrator.
