Review quality measures and open care gaps

See how your population performs on CMS eCQM-aligned quality measures, find the members with an open gap, and turn a gap into a care-gap case.

Before you start​

  • You need the Manager, Nurse Care Manager, or Pop Health Manager role.
  • In the left menu, go to Quality & Outreach → Quality Measures.

Steps​

  1. Read the measure cards. Each client-shippable measure shows a rate (rounded to one decimal), the spec it is aligned with, the steward and when it was evaluated. A measure with no stored evaluation shows an em dash and Not evaluated, never 0%.
  2. Click View members on a card and choose a population: Open gaps, Denominator, Numerator, or Exclusions.
  3. Click Load names for this page to fill the Member, PCP, date of birth and Coverage columns from the patient record.
  4. Type in Filter by name, member id or PCP to narrow the rows on this page, then click Export CSV to download them.
  5. Click Create care-gap case on a row. The case lands in Care Gaps & Outreach.

Tips / Troubleshooting​

  • Rates come from the analytics registry's latest stored run; nothing is recomputed when the page loads.
  • Creating a case twice for the same member, measure and period does not open a duplicate.
  • The filter only searches the page you loaded, not the whole population. Use Previous / Next to page.
