Analyze readmissions
Draft — 2026-09-26
Drafted from the screen's code on main (kareIntelligence 7455702). Not yet reviewed against a live session. Roles listed are the defaults the app's role matrix (@collabkare/ability 0.12.0) gives this screen's route; your organization's role overrides can change them.
Utilization → Readmissions shows 30-, 60- and 90-day inpatient readmissions by PCP and diagnosis category. Index admissions are modeled from the mart's stays.
Tabs
- Overview — index admissions, the 30-day and 90-day readmit rates, the average days from index discharge to first readmission, the rate over time, and a Clinical measures (CR lane) strip.
- Windows & Drivers — when readmissions happen, in mutually exclusive windows (each readmission is counted once, in its earliest band), any-cause against same-diagnosis by window, and the index diagnosis categories driving readmissions.
- Providers — the readmission rate by primary care provider against the filtered blended rate.
- Trending — year-over-year rates by window and the monthly readmission rate.
- Patient Level — patients in scope and a member drill sorted by all-cause readmissions.
Use Readmission match to switch between Any cause and Same diagnosis (an ICD-10-CM category match on the first readmission). The denominator never changes with the scope.
Things to know
- The 30 / 60 / 90 counts in the member drill are exclusive bands.
- High rates on tiny panels are noise — weigh admission counts first.
- Providers appear as PCP IDs; the mart carries no provider names.
- Member drills are capped at 50 rows and carry member IDs only.
