Upload and manage patient documents

Store scanned forms, insurance cards, outside records, and other files against a specific patient so the whole care team can find them later.

Before you start

  • You need patient-directory access (Front Desk Staff, Patient Access Rep, Practice Manager, Billing Specialist, or Admin).
  • Go to Practice Management → Documents in the left menu.
  • The patient must already be registered in your practice.

Steps

  1. At the top of the page, click Select a patient. A patient search dialog opens.
  2. Search for and select the patient. Their name appears in the patient bar at the top — everything you do on this page now applies to that patient.
  3. Click Upload (top right).
  4. In the upload dialog, choose the file and fill in the document details, then confirm. The new document appears in the patient's list.
  5. To work with existing files, use the document list — you can open and review any document uploaded for this patient, whether it was added here or in another CollabKare app.
  6. To switch to a different patient, click Change in the patient bar; click Clear to drop the patient context.

Tips / Troubleshooting

  • Nothing on the page? You haven't picked a patient yet — documents are always patient-specific, so the list stays empty until you select one.
  • The storage meter near the top shows how much of your organization's document storage is in use.
  • Documents uploaded here are visible to authorized staff in other CollabKare apps too — you don't need to upload the same file twice.
