Complete an assessment during a visit
Run standard screening questionnaires — PHQ-9, GAD-7, PRAPARE, AUDIT-C, and Morse Fall — during a video visit. Submitted answers and scores are saved to the patient's chart automatically.
Before you start
- You must have an active visit; assessments are tied to the current visit.
- Open the Assessments screen either from Assess in the top ribbon, or from the side panel: Assess tab → Start Assessment.
Steps
- On the Assessments screen, pick a questionnaire from the tab strip: PHQ-9, GAD-7, PRAPARE, AUDIT-C, or Morse Fall.
- Work through the questions with the patient. Some questions appear only when an earlier answer makes them relevant, and some fields pre-fill from the patient's record — review anything pre-filled before submitting.
- Answer all required questions. If any are missing, a banner tells you how many remain before you can submit.
- Select Submit.
- Confirm the green banner that says the questionnaire was saved and shows how many results were written to the chart. The form then resets, so you can move to another questionnaire tab or return to the visit.
Tips / Troubleshooting
- If you see No patient in context — assessments will load read-only, the visit session lost its patient link. Return to the queue, reopen the visit, and try again.
- If saving fails, a red banner explains the problem — your answers stay on screen, so fix the issue (usually a connection hiccup) and select Submit again.
- Completed and in-progress screenings, with completion percentages, are listed on the Assess tab of the visit side panel, so you can confirm the result landed without leaving the call.
