Write and acknowledge a shift handoff

Shift handoff lists your beds (or the whole unit if you have none) with the latest nursing handoff in SBAR form and the physician's sign-out for each patient.

Write a handoff

  1. Open Shift handoff and click Write handoff (or Update handoff) on the patient's row.
  2. Fill in Situation, Background, Assessment and Recommendation. If there is an earlier handoff, the form is pre-filled from it so you edit rather than retype.
  3. In Handing off to, pick the oncoming nurse from the unit roster or type a name (optional).
  4. Set Priority: Routine, Urgent — watch closely or STAT — unstable.
  5. Save. At least one SBAR section is required.

Acknowledge a handoff

When you receive a patient, read the handoff and click Acknowledge. The row then shows who acknowledged it and when. You cannot acknowledge your own handoff.

Tips / Troubleshooting

  • A handoff marked older than a shift needs updating.
  • The violet Physician sign-out line is the physician's handoff from kareMD, shown for context.
  • Urgent and STAT handoffs show a coloured tag on the row.
